SoDo Pet Intake Form

Welcome to Seattle Canine Club. Please fill the following form with the information about your pet. We will review your information within one business day and get back to you to schedule the next step. We require Rabies for grooming and all others for Daycare/Boarding. If you are looking for Grooming only, fill the fecal date with the same as Rabies. Please provide any supporting file to validate vaccines. Please note we require credit cards on file to minimize no shows appointments.
